Diagnose and treat mental and behavioral health conditions. Provide supervision as needed to Mental Health Nurse Practitioners, Psychologists and social workers including psychosocial assessments, treatment planning, short-term, solution-focused counseling, education and referral for resources and benefits, and case management. Develop program content and organize & lead activities planned to enhance social development of individual members and accomplishment of group goals. Collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan. Interviews patients to assess social and emotional capabilities and determine nature and extent of symptoms. Orders and performs diagnostic tests and therapeutic procedures and interprets results; provides follow-up care. Counsel patients on medications and educate on treatment plans. Develop and implement patient management plans and assist in continuity of care through the Primary Care Management Module (PCMM). Evaluate records from external providers and coordinate co-managed care per VA rules. Conduct individual, group, family and marriage counseling/therapy. Address advance directives and organ donation in accordance with standards. Input patient information into VISTA/CPRS; complete clinical reminders at each visit. Support co-managed care with non-VA providers and facilities; function as case manager as needed. Provide patient and family health education focused on self-management, prevention, wellness, and healthy lifestyle choices. Maintain confidentiality of veterans’ data; comply with VA and company training requirements. Fulfill compliance requirements of OIG, JC, EOC oversight, lab compliance, and related items. Focus on achieving excellent clinical outcomes per VA guidelines; provide excellent customer service to veterans, families, colleagues and clinic visitors. Participate in clinic outreach events; support patient enrollment and retention in appointments and follow-ups. Support new clinical or operational initiatives. Qualifications
Doctor of Medicine or Osteopathic Medicine Board certified in Psychiatry Unrestricted medical license in the state of employment or applicable VA contract requirements Minimum 1-3 years\' clinical practice with adult patients (exact requirements vary by VA contract) Current, unrestricted DEA registration Credentialed and in good standing with the Veterans Health Administration (VA) Current BLS (ACLS as required by contract) Proficiency in written and spoken English Strong computer skills, including EMR experience Energetic and optimistic demeanor Safety sensitive position Qualified applicants will receive consideration without regard to race, color, religion, sex, national origin, sexual orientation, gender identity, disability, or protected veteran status.
