(3) 7p-7a shifts every two weeks, every 4th weekend, holiday rotation, 0.45 FTE Job Summary:
Coordinates patient admissions, discharges, and transfers, acting as a liaison between healthcare providers to ensure seamless patient care and appropriate placement. Manages patient triage calls and facilitates consultations. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License. Essential Functions: Coordinates patient admissions, discharges, and transfers following hospital protocols. Acts as a liaison among healthcare providers for seamless patient care. Manages the Primary Care Phone Triage Line, assessing urgency and determining appropriate care. Facilitates pediatric specialist consultations, including telemedicine. Participates in professional development and quality improvement projects. Ensures accurate documentation and communicates pertinent information. Education:
Bachelor’s Degree in Nursing from an approved school. Licensure:
Ohio RN license. Certifications:
Current CPR required; willing to obtain NRP, PALS, ACLS, APHON as needed. Skills:
Excellent communication, organizational, critical thinking, and computer skills. Experience:
At least 3 years of clinical experience, pediatric experience required. Physical Requirements:
