The Clinical Director (RN) is responsible for overseeing and directing the patient care operations of the home care facility under the direction of the Home Health Executive Director. Coordinates and completes assigned projects to effectively support the immediate and long-range objectives of the company. Oversees the eligibility of patients referred to home care services, plans the services to be provided to patients, and supervises their total home health care. Implements and maintains administrative practices, agency philosophy, goals, and policies which assure compliance with applicable state and federal regulations. Enhances the profitability of the agency and engages with the team to assure the quality of services delivered. Supervises and ensures high quality patient care services throughout the episode of care, including referral coordination, ongoing needs assessment, and the development, implementation, and updates of the patient\'s individualized plan of care; ensures coordination with physicians, families, and other clinicians; communicates with staff through conferences and mandatory in-services, including monthly staff/safety meetings. Supervises patient care to ensure safe, economical, and efficient delivery, adheres to nursing and medical standards, and complies with federal/state regulations, Joint Commission standards, payer criteria, and LHC policies; ensures care according to the physician\'s plan of care. Supervises agency staff performance and uses the clinician scorecard and annual performance evaluations to monitor and assess staff. Ensures that only qualified personnel are hired; oversees licensure verification, certification, and credentialing as required by law and policy prior to employment and annually thereafter. Assists with staff recruitment to maintain adequate staffing; operates within the staffing model. Serves as a member of the Management Team and QAPI Team, implements Performance Improvement Projects and annual strategic agency planning, and maintains an infection control program to prevent and control infections and communicable diseases. Participates in annual strategic agency planning and ensures follow-up on required action plans. Conducts at least monthly meetings with the medical director and handles medical director invoices for timely payment. Drives operational metrics (e.g., productivity, VBP, SVP, ACH, claims alerts, late EMS, orders audit, and agency financial statements) to achieve goals related to efficiency, growth, quality, and finances; follows up and implements action plans as needed. Acts as Emergency Coordinator in the absence of the Executive Director/Alternate Executive Director during emergencies and ensures appropriate plan execution. Communicates effectively with customers to represent the agency and ensures timely investigations of patient, caregiver, and referral source concerns, with follow-up to ensure resolution; maintains confidential documentation and collaborates with the sales team to grow the business. All other duties as assigned. Qualifications
If Clinical Director is an RN, must have current RN licensure in the state of practice. Current CPR certification required. If Clinical Director holds another clinical discipline (PT/OT/SLP), must have current applicable license in the state. One year of supervisory and/or management experience in home health setting or related healthcare setting preferred.
