Patient Care Coordinator
Coordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients. Job Duties: Provides discharge information. Partners with providers to ensure a smooth transition at discharge to home or other facilities as required. Ensures additional resources are made available for the patient as needed. Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patients plan of care. Maintains systems and processes to assure timely follow-up and surveillance of a defined group of patients. Serves as liaison with clinical and administrative personnel. Establishes point of contact for referring providers to create immediate access to provider templates. Develops and implements efficient patient navigational procedures. Investigates patient concerns including billing, appointment access, perception of care, physician issues and communication problems. Develops, maintains, and audits standard operating procedures of patient access and utilization process. May provide administrative support as needed. Work is typically performed in a clinical and office environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.
