What You Will Do: Implement customized care plans that address the whole person in mind, body and spirit. Treat clients like family and strive to enhance their quality of life.

Essential Functions

• Deliver exceptional care to the client in their home
• Assist with comprehensive personalized weekly schedules and care plans
• Complete daily tasks in a safe and professional manner (ADLs)
• Provide companionship and ongoing social & emotional support
• Assist with personal care activities including hygiene, mobility, incontinence care, light housekeeping, feeding and meal preparation
• Maintain safety, order, and client enjoyment at all times
• Provide transportation to and from appointments as needed
• Document task completion and report changes in the client’s physical condition, mental capability or behavior
