Salary Range:
Master’s Level Unlicensed: $58,011 - $69,618 annually. Master’s Level Licensed: $66,040 - $79,217 annually based on relevant years of experience. Location:
Community-based, some onsite and remote work Status:
Full Time, Hourly, 40 hours per week-Salaried
Role Overview- The Home Bound Services team members are an integral part of SummitStone and our Northern Colorado community, providing in-home (independent living, assisted living facilities, and skilled nursing facilities) therapeutic services to vulnerable adults unable to access traditional outpatient behavioral health services. We are looking for individuals who enjoy working within a team, are versed in Evidence Based Treatment practices, and those seeking to grow their clinical skills in a fast-paced environment. Essential Duties: Provide in-person, direct client care to include on-going behavioral health related screenings and assessments, individual therapy, and crisis intervention. Develop and monitor service plans, monitor treatment progress, document interactions, and maintain clinical records and other reporting data for clients with mild to severe mental illness and/or substance use disorders. Coordinate with internal as well as external supportive services to deliver needed services to clients. Collaborate with other staff in a multidisciplinary treatment team model by providing, planning, developing, implementing, and evaluating clinical services. Develop and monitor service plans, treatment progress, and maintain timely clinical documentation and other reporting data. Have a strong understanding of how to adapt service delivery to meet the client’s needs and how to engage collaterals and caregivers.Maintain superior customer service for internal and external customers by providing information and answering questions/requests in timely manner. Perform other duties as assigned. Required: Master's degree in social work, counseling, psychology, or related field from an accredited College or University
and
registered with DORA as a Candidate working towards licensure as LPC, LCSW, or LMFT. Experience or coursework in working with adults and aging populations. Proficiency in using computers and telehealth systems. Addictions Counselor training/certification or willingness to pursue. Access to a personal vehicle, valid Colorado Driver’s and ability to drive to client homes within Larimer County. Mileage is reimbursed by the agency.
Highly Desired At least one year of experience in the behavioral health field. Licensure (LPC, LCSW, or LMFT), CAS or LAC Experience with Trauma-Informed Care 1 year of experience providing play therapy and family interventions Bilingual/Bicultural Some positions include driving a company vehicle and require a valid Colorado driver’s license. There are opportunities within our team-based model to attend company-sponsored training programs, complete certifications (such as CAT or CAS) and to learn from seasoned colleagues daily. SummitStone provides clinical supervision toward licensure at no cost to the employee and reimburses any fees required for licensure.
