Journey Hospice is seeking a dedicated per diem RN Case Manager to serve clients in Essex County. The Hospice RN oversees a primary caseload, providing skilled nursing care to patients, their families, and caregivers, in accordance with physician prescriptions and compliance standards including Medicare Conditions of Participation, New Jersey laws, and agency policies.

The responsibilities include:

1. Conducting initial and comprehensive nursing assessments of patients.
2. Reassessing and updating the plan of care in collaboration with the Interdisciplinary Team (IDT).
3. Implementing and revising care plans to meet patient needs.
4. Educating and supervising patients, families, and staff regarding nursing care and patient needs.
5. Collaborating closely with the IDT to ensure all care needs are met.
6. Supervising Hospice Aides and Homemakers.

Qualifications include:

• Graduation from an accredited RN program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• Valid NJ RN license and a valid driver’s license with reliable transportation.

Skills and Experience:

• Professional attitude with effective communication skills.
• Positive, optimistic approach and team-oriented mindset.
• Minimum of 1 year of general nursing experience, with hospice, home health, or critical care preferred.
• Experience in pain and symptom management is a plus.
• Understanding of hospice philosophy and principles of death and dying.
