We are looking for an experienced home health and hospice RN to provide excellent care to our community. In this role you will organize and direct home care and hospice services and coordinate care amongst disciplines and intake for our weekend team.
Excellent salary and benefit pack available.
3x10 hour weekend shifts with on call (Saturday through Monday).
Patient Care
Primary Duties and Responsibilities
- Completes an initial assessment of patient and family to determine home care/hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan which establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive, rehabilitative and comfort focused nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient and caregiver as appropriate per assessment and plan.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Primary resource person for the weekend multidisciplinary team including nurses, therapists, HHAs an intake.
- Will utilize weekend manager on call as needed for an additional resource.
- Will evaluate new referrals in collaboration with the intake to determine appropriateness for home care or hospice services as necessary.
- Assist with patient prioritization of needs and communicate to team members on any change in needs.
- Graduate of an accredited school of nursing.
- 1-2 years of recent patient centered care/nursing experience.
- Current state licensure, CPR certification, and a valid driver’s license.
- Bachelor's degree in nursing with one year of home health care experience preferred.
- Strong observation, communication, and problem-solving skills.
- Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.
