Overview

CNA - Hospice Aide - Cornerstone Hospice at Chapters Health System

Responsibilities

  • Performs personal care, homemaker assistance and defined nursing procedures under RN direction and supervision.
  • Maintains competencies and provides patient/caregiver services according to the Plan of Care and Company policies.
  • Practices safe body mechanics when moving, positioning and transferring patients.
  • Provides caregiver relief as part of the Plan of Care and assists in maintaining a clean and safe home.
  • Transports supplies, lab specimens, documents and other items as requested by the RN.
  • Under RN direction, may perform activities that promote activities of daily living; reports observations to the RN to contribute to the Plan of Care.
  • Communicates significant issues or symptoms to the RN or Clinical Manager; performs other duties as assigned.
  • Participates in non-clinical activities as needed to support quality and performance improvement initiatives.

Qualifications

  • High school diploma or GED or an equivalent combination of education and experience
  • Current CNA certification in the state of employment
  • Minimum six (6) months of CNA or Home Health Aide experience
  • Active Basic Life Support (BLS) for Healthcare Professionals from American Heart Association or Red Cross
  • For Hospice House CNA in the Hospice House, obtain certificate prior to specified deadlines if applicable

Skills & Requirements

  • Mobile driver with valid driver's license and automobile insurance per Company policy
  • Reliable transportation to meet work schedule
  • Ability to use equipment with visual and auditory mechanisms
  • Proficient in English (written and verbal) communication
  • Ability to manage emotional stress of caring for ill patients and families
  • Ability to perform essential functions and physical requirements (lifting, bending, pushing/pulling, kneeling) with or without reasonable accommodation
  • Must be able to access homes or facilities in the assigned area (stairs, narrow spaces, heat, lack of air conditioning)
  • Willingness to have contact with individuals with communicable diseases

Physical Demands

Post-offer/pre-placement requirements; see job description for essential functions, including seeing, hearing, talking, walking, using hands; frequently required to transfer/ambulate patients, stand, sit, reach, lift, bend, push/pull, balance; occasionally stoop/crouch, kneel, climb stairs.
