Responsibilities
- Provide supportive services and counseling on healthcare and home care programs and services.
- Serve as a member of the interdisciplinary team to assist with social, emotional and economical concerns of patients, residents, families and caregivers.
- Coordinate and plan programs to enable optimal functioning; provide crisis intervention; educate families on medical implications.
- Develop plans of care by obtaining resources from social, health and human services agencies; provide referrals and current information about available programs.
- Participate in care conferences and transitional rounding; advocate for patients/clients/residents and families/caregivers; demonstrate critical thinking and effective communication.
- Respond to suspected abuse, neglect or violence per NASW Code of Ethics and applicable state laws.
- Possibly provide social services for donors and transplant recipients depending on department.
Qualifications
- Bachelor’s degree in Social Work from an accredited program; Master’s degree preferred.
- Valid driver’s license and good driving record.
- Healthcare and/or mental health hospital experience preferred.
- Basic Life Support (BLS) certification may be required within six weeks of employment depending on location; multi-state licensure privileges may be required.
- Need licensure in good standing in the state of practice:
- In Iowa: Licensed Bachelor Social Worker (LBSW)
- In Minnesota: Licensed Social Worker (LSW) or other allowed credential per Minnesota statute
- In North Dakota: Licensed Baccalaureate Social Worker (LBSW)
- In South Dakota: Social Worker license (SW)
- Licensure or certification as a social worker in the state where the location is located, if required by the state licensing board
