Responsibilities

• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Be part of a team that appreciates, supports and relies on each other in a positive environment.
• Support patients in understanding their rights and responsibilities and guide them in managing the challenges of dialysis care.

Requirements

• Meets all state required regulations to practice in the dialysis setting.
• Master’s degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills (MS Word, Excel, PowerPoint and Outlook) and functional proficiency with DaVita-specific applications within 60 days.
