The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Join a service that values safe, compassionate, and person-centred healthcare. NBMLHD is seeking a dedicated Social Worker to join our multidisciplinary therapy service. This is an excellent opportunity for a social worker to gain experience in supporting clients with chronic and complex health and psychosocial needs. You will strive to bring a holistic approach to patient care as you work with clients to minimise hospital admissions while maintaining optimal health outcomes.

Ready to make a difference? Bring your expertise and compassion to a team that values integrity, collaboration and cultural inclusivity. At NBMLHD, every shift is a chance to meaningfully contribute to your workplace, your colleagues, and your career. Let’s shape the future of healthcare, together. Apply today!
