A board-certified or board-eligible family or internal medicine physician with a geriatric fellowship provides full-time outpatient care primarily in skilled nursing facilities in Southbay San Diego. Responsibilities include patient admissions, rounding, discharging, leading family conferences, and participating in interdisciplinary team meetings, with a focus on coordinated geriatric care.

Requirements/Preferred Experience
Must be board eligible/board-certified physician in family medicine or internal medicine with a geriatric fellowship highly preferred to join our Sharp Extended Care (SEC) Department
Coverage at two skilled nursing facilities
1.0 FTE (SNF- Southbay San Diego)
Spanish Speaking preferred
Duties Of Position
Provide care in a skilled nursing facility, including admissions, rounding, and discharging patients for both Sharp Rees-Stealy and Sharp Community Medical Group.
Schedule: Monday-Friday 8am to 5pm – expected to provide eight hours of patient contact per day.
1 weekend coverage a month (Friday 6pm-Monday 7am)
Twice a month weeknight call (5pm -7am).
