Description

Comunilife Inc. is looking for aWellness Coordinator/Health Educatorfor its congregate residential program in the Bronx, La Central. The Wellness Coordinator for La Central provides psychoeducation and support, referrals and linkages to community resources for clients, ensuring Responsibilities include Responsible for planning and coordinating special events and holidays activities. The Counselor will also develop and implement weekly recreational activities including field trips, outings, arts & crafts, games, music and other hobbies. Schedule:

Monday - Friday 9am - 5pm

Essential Duties:

Conduct needs assessments

Establishing a network of community resources to facilitate wellness activities

Recommend appropriate activities, lead and co-lead activity groups and encourage clients’ participation in community events

Provide wellness-related counseling and monitor clients’ progress with their goals

Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented

Qualifications

Bachelor’s Degree in Human Services, Education or a related area

Licensed Practical Nurse (LPN) NYS license

Two (2) years’ work experience in benefits and entitlements with extensive knowledge of community resources

Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus

Bilingual skills in English/Spanish a plus

Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness

Excellent oral and written communication skills; proficiency with MS Office and Google product suites

Ability to perform and manage in a team approach; excellent interpersonal skills

Ability to handle complex challenges and balance multiple demands on time and attention
