1) How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm? Yes – possible for occasional on call needs 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Both 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Potentially, pending on performance 6) What region are you in? Home health or Hospice? Home health 7) What’s creating the request? - work load 8) To which location will this person report for assignment, full address, Including the OU (Location Code) 10805 Sunset Office Dr Suite L 101 St Louis Mo 63127 1302 / I83 Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9) How many miles per week will the consultant travel? 30/40 a day?
