Eden Meadows, a skilled nursing facility, is seeking a dedicated Certified Nursing Assistant (CNA) with Medication Aide certification to join our care team. This dual role combines direct resident care with safe and accurate medication administration under the supervision of licensed nursing staff.

Primary Responsibilities

Resident Care

  • Aids activities of daily living in an environment that promotes residents’ rights, dignity, freedom of choice and individuality, including bathing, grooming, dressing, toileting, and feeding.
  • Safely assist residents with mobility, positioning, and transferring using proper techniques and equipment.
  • Monitor and document vital signs, food and fluid intake, and any notable changes in resident condition.

Medication Administration

  • Accurately administer oral, topical, and other approved medications per physician orders and facility protocols.
  • Document medication administration in the electronic medical record (EMR) system according to state and facility guidelines.
  • Observe for and report any adverse reactions or changes in resident conditions to licensed nursing staff immediately.

Qualifications

  • Active CNA license in the state of Wisconsin or graduate of a Nursing Assistant class who will take the Federal test within 120 days of class completion.
  • Medication Aide certification recognized by Wisconsin licensing board (e.g., CMA, MA-C, CNA II with med-passing endorsement).
  • Previous experience in a geriatric, psychiatric, skilled nursing, or long-term care facility preferred.
  • Proficiency in documentation, especially with electronic health records (EHR).
  • CPR/BLS certification (required or ability to obtain upon hire).
  • Strong attention to detail, reliability, and ability to work in a fast-paced, team-oriented environment.
  • Physical ability to lift up to 50 pounds and meet the demands of direct patient care.

Team Collaboration & Communication

  • Work closely with nurses, therapists, and other care team members to support comprehensive resident care plans.
  • Participate in shift hand-offs and ongoing training sessions.
  • Communicate effectively with residents, families, and staff in a respectful and compassionate manner.

Supervisory Responsibilities

  • This position has no supervisory responsibilities.

Compliance & Safety

  • Follow all state and federal regulations, including HIPAA, infection control, and medication administration standards.
  • Maintain clean, safe, and organized work environments in resident rooms and medication areas.
  • Assist with audits, supply checks, and other regulatory readiness tasks as assigned.

  • No travel is expected for this position.
