We’re currently seeking a Nurse Navigator to join our care team. This position is ideal for a Health Care Professional with clinical and navigation experience who thrives in a fast-paced healthcare setting and enjoys meaningful patient engagement.

Position Summary

The Nurse Navigator supports patients in ensuring seamless care navigation and promoting health equity through outreach. The Clinical Navigator also acts as a liaison to coordinate services through plan benefits, conduct follow-ups, and support patients in navigating complex care journeys.

Key Responsibilities

• Provide guidance and navigation support for patients based on plan benefits.
• Coordinate referrals, follow-up appointments, and patient communication
• Participate in quality improvement initiatives and maintain compliance with regulatory standards
• Serve as a patient liaison, ensuring clear communication with referring providers and coordinating care transitions (e.g., oncology referral, listing, and discharge)
• Conduct outreach assessments to identify patients at risk for adverse outcomes
• Represent Northwind Health at offsite clinics and community outreach events

Qualifications

• Bachelor’s Degree (required)
• 3+ years of clinical experience (required)
• 1+ year of care navigation experience (preferred)
• Proficiency in electronic medical records and general computer use
• Strong interpersonal and communication skills
• Excellent customer service and organizational skills
