You will be an integral part of the multidisciplinary team, providing both essential clinical expertise and managerial oversight to staff supporting eligible Medi-Cal beneficiaries experiencing homelessness and complex health needs. This role is a dynamic blend of leadership and direct clinical practice. You will serve as a lead overseeing and supporting a team of approximately 10-15 field-based staff.
Leadership & Oversight:
- Oversee and support a multidisciplinary team including Care Managers, Case Managers, and Community Health Workers, providing clinical expertise and ensuring high-quality service delivery.
- Mentor and guide staff to provide intentional case management support, helping members transition from high-intensity services to more independent, recovery-focused care.
- Contribute to team strategy to help staff move members toward obtaining and maintaining stable housing.
Direct Clinical & Therapeutic Support:
- Provide therapeutic supportive services addressing members' mental health, functional, and substance use barriers to housing retention.
- Conduct crisis and suicide risk assessments and provide appropriate interventions, completing critical incident reporting within 24 hours.
- Offer crisis intervention services to enhance members' independent problem-solving, coping skills, and self-management.
- Support team members in conducting necessary assessments, including comprehensive biopsychosocial assessments for new members.
- Perform follow-up with members concerning mental health and safety concerns identified by assessments or team feedback.
Minimum Qualifications:
- Bachelor's Degree in Social Work or Psychology
- Proven managerial experience or experience leading teams in a case management or social service setting.
- One (1) year of experience in intensive case management with vulnerable populations, specifically in: homeless services, veteran services, mental health, substance use, or co-occurring disorders.
Required Experience & Skills:
- Vast experience in housing services.
- Strong skills in carrying out case conferences and utilizing a holistic approach in housing retention services.
- Expertise in psychosocial assessments and working with members who have co-occurring or tri-morbid conditions (mental health, physical health, and substance use).
- Experience with field-based case management interventions delivered on the street, in shelters, or in the member's home.
- Familiarity with evidence-based practices, including: Housing First , Critical Time Interventions , Harm Reduction , Motivational Interviewing , and Crisis Intervention .
- A high level of tolerance and empathy for individuals with urgent and multiple health needs.
- Ability to execute and maintain confidential information according to HIPAA standards.
Role Requirements
- Schedule: Monday - Friday, 8:00 AM – 5:00 PM.
- Flexibility: Willingness and ability to work flexible hours, which may include evenings or weekends as needed.
