Job: Occupational Therapist (OT) Home Health - Full Time / Part Time (Per Diem) Location: San Francisco County, CA About Us

We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full Time or Part Time (Per Diem) Occupational Therapist (OT) in San Francisco County, CA. Position Overview
We are seeking an experienced, self-driven Full-Time / Part Time (Per Diem) Occupational Therapist (OT) to join our Home Health team and provide exceptional care throughout the region. As an OT, you will evaluate patients\' health, create and implement care plans, and track their progress to maintain the highest standards of care. You\'ll work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field. Responsibilities
Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient\'s home. Regularly evaluates and re-evaluates patient status. Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing. Initiates, develops, implements and revises the patient\'s plan of care (POC). Addresses all problems identified in the POC. Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient\'s condition. Communicates with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation according to company policy using a company-provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements
Current OT License to practice in California - Required Experience in Home Health or Hospice care - 1 Year Required Current CA Driver\'s License Able to travel within a 15 - 25 mile radius on a regular basis. Benefits
