The Care Manager (CM) works under the direct supervision of the Care Manager Supervisor and plays a critical role in achieving the overall goals for coordination, integration, and partnership in the delivery of care to children, youth, and families with complex medical, developmental, and behavioral needs between the ages of 0-21. The Care Manager is the central point of contact for the family and the child/youth, to ensure that their needs and services are met with positive regard. Duties And Responsibilities

Develops and implements a person-centered, comprehensive Plan of Care (POC) for the member, in partnership with his/her caregivers and service providers. Completes the initial and annual Child Adolescent Needs and Strengths-New York (CANS-NY). Based on the complexity level outcome of the child/youth, the care manager must meet the specified minimum contact and face-to-face requirements centered around the Core Services for billing as per the Department of Health (DOH) mandates. Conducts all other required additional assessments within prescribed timeframes including but not limited to the Comprehensive Assessment, Crisis Plan, Plan of Care Updates, and Home and Community Based Services (HCBS) Level of Care (LOC), and eligibility determination for the provision of services when appropriate. Completes all program/case relevant documentation in the Electronic Medical Record (EMR) within the assigned deadlines. Requests and obtains the necessary documentation in a timely manner from service providers and/or families. Partners with multi-disciplinary team to determine service needs and actively participates in ongoing communication with all partners in care. Participates in service coordination meetings and team conferences. Provides ongoing collateral contacts and follow up on coordinated services. Facilitates the transitional care process and discharge planning, when applicable. Attends and participates in individual and group supervision. Carry a caseload as determined by the complexity levels of the cases (20-30 cases) with the ability to travel to complete face-to-face visits via car or public transportation. Caseloads will be located in or near designated borough. Attend and participate in ongoing staff development training to enhance skills needed to effectively meet the demands of the Care Manager position. Perform all other duties as assigned. Requirements

Experience in medical service provision and care preferred. Must possess excellent verbal, written, organizational and communication skills. Demonstration of dedication to quality care and customer service. Computer proficiency in Excel, calendar, and Word documents. Ability to perform the essential functions of the job with or without a reasonable accommodation. Bilingual a plus. Qualifications/Requirements

Bachelor’s degree, 2 years relevant experience and 1 year of Microsoft 365 experience required. NYS Driver’s License or NYS ID required. Our Agency operates in 2 locations: Staten Island and Brooklyn. The ability to travel between sites is required. Benefits
