Essential Functions Regularly conducts street-based outreach to identify and engage transition-aged youth (18-24 years old) experiencing homelessness or housing instability by driving to various communities and walking through locations where unhoused individuals congregate. Facilitates entry/exits of clients in hotel-based shelter rooms (2-3 rooms) providing case management and monitoring cleanliness and compliance with hotel rules. May work evenings and weekends as required for program goals.

Builds and maintains rapport with youth through consistent, respectful, and nonjudgmental engagement while maintaining appropriate professional boundaries. Links individuals to services when needed.

Completes timely safety assessments, intakes, housing assessments, and individualized service plans focused on client goals. Assesses and enters clients on Coordinated Entry lists for DuPage and Kane Counties where appropriate.

Provides strengths-based, trauma-informed case management services including navigation of housing options, public benefits, mental health care, substance use treatment, and other supportive services.

Partners with shelters, drop-in centers, clinics, and other community resources to coordinate care and maintain knowledge of new resources.

Completes Homeless Verification documentation for clients and other partner agencies within reason.

Maintains accurate and timely documentation using HMIS and other tools to track outreach and case management data for reporting and program evaluation.

Assists clients with obtaining resources to meet basic needs including survival gear, hygiene items, transportation assistance, and harm reduction supplies by researching internally and externally for these resources.

Participates in case conferences and multidisciplinary teams to support client progress.

Provides appropriate and timely response to crises utilizing de-escalation techniques and safety planning as needed.

Immediately notifies leadership of crisis events and outcomes.

Participates in community events such as resource fairs, tabling opportunities, and public awareness campaigns to expand outreach and build partnerships with other service providers. Events may occur on evenings and weekends.

Represents the organization in a professional and inclusive manner, upholding values of equity, respect, and dignity for all people, with particular attention to affirming LGBTQ+ youth and communities of color.

Performs other duties as assigned.

Knowledge And Ability Requirements Proficient in applying trauma-informed care principles to engage and support youth experiencing homelessness, ensuring services are sensitive to past experiences and current needs.

Skilled in utilizing de-escalation techniques and crisis intervention strategies to manage high-stress situations effectively, ensuring safety of clients and staff.

In-depth understanding of local community resources, including housing options, healthcare services, employment programs, and educational opportunities, to connect clients with appropriate support systems.

Ability to work effectively with individuals from diverse backgrounds, including LGBTQ+ youth and communities of color, demonstrating respect and understanding of cultural differences.

Experience conducting assessments, developing individualized service plans, and maintaining accurate and timely documentation using systems like Microsoft Office and HMIS.

Excellent verbal and written communication abilities, with a talent for building rapport and trust with clients, colleagues, and community partners.

Ability to work independently in the field while also collaborating effectively within a multidisciplinary team to achieve client goals and program objectives.

Demonstrated flexibility in adapting to changing circumstances and environments, with strong critical thinking skills to address challenges and find solutions.

Education And Experience Bachelor's degree in Social Work, Psychology, or related field preferred; equivalent experience (including lived-experience) may be considered.

Minimum 1–2 years of experience working with homeless or at-risk youth, preferably in an outreach or field-based setting.

Knowledge of harm reduction, trauma-informed care, and youth development principles.

Demonstrated commitment to equity and inclusion, with an understanding of the unique barriers faced by LGBTQ+ youth, youth of color, and other marginalized populations.

Ability to work independently in the field, manage time effectively, and maintain strong boundaries.

Strong communication and interpersonal skills; able to build rapport with diverse populations.

Bilingual (Spanish, ASL, or other) preferred but not required.

Must have reliable transportation, valid driver’s license, and ability to travel throughout the service area.

Physical Demands And Other Requirements

To perform this job successfully, an individual must be able to perform the essential duties listed in the job description satisfactorily. The role requires talking, hearing, sitting, standing, walking, using hands, reaching, and operating standard office equipment. Moderate lifting may be required (up to 20-30 pounds). Valid driver’s license, auto insurance, and ability to safely operate a motor vehicle are required. The work environment is typically low to moderate noise.
