The Cardiologist is a licensed/board-certified professional who plays a key role as part of the clinical operations team, providing direct patient care and assessments, primarily in the ambulatory health care center setting. Responsibilities include diagnosing and treating cardiovascular diseases, taking medical history, performing physical exams, developing care plans, providing education, making specialty referrals, and documenting progress notes according to ChenMed standards for quality, service, productivity, and teamwork. The cardiology clinician may participate in clinical rounds and conferences. Essential Job Duties / Responsibilities
Provide excellent inpatient and outpatient cardiovascular care, care coordination, echocardiogram reading, population management of market members, and oversight of PCP cardiovascular skills. Lead a cardiovascular care team that may include Care Promoters, Echocardiogram/X-ray Technicians, and in some cases a mid-level (ARNP or PA) dedicated to cardiology depending on scope and member volume. Assist the Medical Director, Cardiology (MDC) in managing day-to-day clinical and business operations and build positive relationships with providers and key stakeholders for all ChenMed/JenCare/Dedicated Senior Health centers within the market. Consult with the Medical Director, Cardiology to ensure guideline compliance and participate in risk and quality management programs, clinical meetings, and other programs that maximize clinical value and align with business objectives. Manage care of chronic cardiovascular diseases; examine patients, order tests, and develop appropriate care or treatment plans to meet clinical needs for a panel of at least 5,000 patients (with care team support). Assess patients’ ability to withstand stresses of cardiovascular or other surgeries and assist surgeons or invasive cardiologists in management planning, including clearance for surgery. Read and interpret EKGs, chest X-rays, and echocardiograms; order relevant blood tests and ancillary services as needed. Limit external cardiovascular care where high-quality internal care can be provided to members. Prescribe medications and treatments in accordance with patient condition, payer guidelines, and industry standards. Review PCP requests for external cardiology referrals and participate in delegated utilization authorizations; determine urgency of follow-up and appointments. May oversee cardiology operations of 2-5 centers in the market under supervision of the MD, CVC. Exercise final medical judgment in health care decisions for paneled patients. Provide patient education related to heart disease, wellness, prevention, and early detection; support market CHF clinics and related initiatives. Assist with training and hiring of dedicated cardiology Care Promoters and NPs/PAs, and mentor PCP physicians and other clinicians in cardiology management in the PCP setting. Participate in cardiovascular team meetings, daily huddles, and market/huddle discussions; contribute to quality and performance initiatives and cost-reduction strategies; build relationships with internal and external physician stakeholders. Perform other related duties as assigned and modified at manager’s discretion. Knowledge, Skills And Abilities
Thorough knowledge of pharmacological agents used in patient treatment. Familiar with standard concepts, practices, and procedures within the Cardiology field. Superior clinical skills in managing seniors with complex medical needs across care environments. Excellent communication skills and ability to build relationships with external organizations and primary care teams. Strong relationship-building with center-based care teams and staff; good analytical and results management skills. Conflict management and resolution skills; experience with population risk management or complex chronic disease care management is preferred. Effective verbal and written communication; professional telephone etiquette. Proficient with electronic medical records and keyboarding skills. Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint) and other productivity tools. Willingness to travel locally, regionally, and nationwide up to 15% of the time. English fluency (spoken and written). Ability to exercise independent judgment in clinical decision-making. Education And Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. Board certification in Cardiology required; other sub-specialty training and board certification may apply. Current, unrestricted license to practice medicine in the state of employment. Current DEA and DPS certificate of registration required. Minimum of 2 years’ experience as a Cardiologist required. Experience with interdisciplinary teams in quality improvement and healthcare leadership is preferred. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within 90 days of employment.
