The Oncology Nurse Navigator 2 is a registered nurse (RN) who works with physician practices and multi-disciplinary teams to promote the effective utilization of services and coordination of care for patients within the Cancer Center service line. The Oncology Nurse Navigator 2 contributes to the assessment of patients' psychosocial, financial and clinical needs throughout the continuum of care ensuring high quality care while supporting quality metrics of the Cancer Center and Rush University Medical Center. The Oncology Nurse Navigator 2 functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies of oncology navigation: Care Coordination, Communication, Education, and Professional Role/Care as well as the domains of the Profession Practice Model: Relationships and Caring, Evidence Based Practice, Technical Expertise, Critical Thinking and Leadership. The Oncology Nurse Navigator 2 care may be focused in medical oncology, hematology, stem cell transplant, gyne-oncology, surgery, radiation oncology, cancer screening or supporting multi-disciplinary clinics. Education/Experience Current State of Illinois Registered Nurse licensure required At least one year of nursing experience Earned nursing degree of baccalaureate level of education or higher Additional Qualifications Oncology nursing experience preferred Oncology nursing certification preferred Physical Demands
Ability to travel throughout the Medical Center and some roles may require travel to other sites of care. Responsibilities
Establishes a caring therapeutic relationship with patients and families while managing a defined patient caseload, utilizing effective communication. Assess patients for social determinants of health; utilize findings to develop a plan of care and mutual goal setting. Serve as liaison for the patient, becoming an expert on the patient and their needs throughout the trajectory of their care from diagnosis to end of life or survivorship. Advocate for patient and families to ensure timely access to treatment and equal access to health care. Collaborate with all members of the health care team to ensure coordination of patient care. Serve as a reference regarding clinical trials and coordinate care with the clinical trial office. Provide psycho-social support and facilitate referrals and transitions of care ensuring effective utilization of internal and community resources. Provide symptom management utilizing guidelines/evidence-based practice from oncology resources in collaboration with providers. Perform role utilizing EBP within specialty established by nationally recognized oncology nursing associations, cancer societies and networks as well as published research and quality documents. Respond to patient questions and needs via My Chart, staff messages, telephone, and other communication channels. Coordinate/schedule admissions, surgeries, laboratory testing, radiographic exams, and infusion; perform basic administrative tasks; establish plan for venous access; perform nursing procedures per policy; assist providers with procedures; assess patient and family learning needs and provide ongoing education; document thoroughly in the electronic medical record. Review medical records for completeness and ensure appropriate scheduling with providers; adjust plan as needed; assess patients in person or remotely to evaluate status and needs; provide telephone triage as defined by guidelines and coordinate care as required. Recognize changes in patient status and respond promptly to optimize outcomes; prioritize complex patient care based on knowledge and experience; assess patient compliance with treatment plan. Demonstrate accountability for professional practice, continuing education, and progress toward goals; establish expertise regarding disease, staging, current treatment options and adverse event management. Serve as liaison between practice and referral sources; participate in quality metric initiatives; assist in tumor board identification, coordination, and patient care plan development; participate in orientation and education of health care team members; support patient-focused care as a key member of the multi-disciplinary team.
