Staff Nurse
to join our Pediatric Home Health team in Monmouth County, NJ. This is your opportunity to be part of a dynamic organization that is committed to moving healthcare forward and making a lasting impact on the lives of mothers and children. Your Role
Provide Exceptional Care: Deliver comprehensive nursing care to mothers, infants, and children in their homes, ensuring their well-being and promoting healthy outcomes. Support New Mothers: Offer education, guidance, and emotional support to new mothers, helping them navigate the challenges of early parenthood. Promote Child Health: Monitor the growth and development of infants and children, providing necessary care and interventions to support their health. Follow the Case Management Model: Provide and follow a case management model of care, ensuring coordinated and comprehensive care for all patients. Perform Assessments: Conduct physical assessments, evaluate the needs of patients, and develop appropriate plans of care. Document Patient Progress: Document all changes in clients’ conditions, update care plans, and track outcomes to ensure effective care management. Educate and Empower: Identify learning needs and plan appropriate teaching strategies for individuals and families to support their understanding and involvement in care. Collaborate with a Multidisciplinary Team: Work closely with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans. Advocate for Patients: Serve as a compassionate advocate for mothers and children, ensuring they receive the care and resources they need. What We’re Looking For
Qualifications: Registered Nurse (RN) with a Bachelor’s degree in Nursing preferred. Experience: 1+ year of experience in pediatric or Maternal Child Health. Skills: Strong clinical skills, particularly in maternal and child health, with the ability to provide patient-centered care. Attributes: Excellent communication and interpersonal skills, with a focus on empathy, compassion, and teamwork. Ready to Make an Impact?
