Charter Healthcare POSITION SUMMARY: The Director of Patient Care Services shall have the overall responsibility for coordination of patient care services, including personnel qualifications, assignments, and supervision. This executive position ensures quality and adequacy of services, acts as a liaison among staff, patients, families, and the community, and ensures compliance with care standards. REPORTS TO:

Executive Director SUPERVISES:

Direct Patient Care Staff QUALIFICATIONS: Education: RN with a bachelor's or higher in nursing or related field, with 3 years recent experience in hospice, home health, or similar, including 1 year supervisory/administrative experience; or RN with 4 years recent experience in similar settings, including 1 year supervisory/administrative. Credentials: Active RN license in the state; hospice, home health, or palliative nursing certification preferred. Core Competencies: Clinical decision-making, performance of nursing functions, knowledge of laws/regulations, communication, staff organization and evaluation, team collaboration, and a valid driver’s license with auto insurance. Functions and Responsibilities: Ensure compliance with laws, regulations, and standards. Maintain qualified clinical staff levels. Develop, interpret, and enforce policies and procedures. Manage budgets and expenditures responsibly. Communicate regularly with staff on organizational and clinical issues. Oversee patient assessments, interventions, and documentation. Participate in budget development. Investigate and resolve problems and complaints. Provide leadership through communication, staff development, and motivation. Establish performance standards and conduct evaluations. Oversee staff orientation and ongoing education. Assist in program planning and development with the Executive Director. Coordinate with Medical Director on patient care. Implement and oversee Quality Improvement programs. Ensure continuity of care and after-hours service quality. Maintain accurate medical records and legal compliance. Visit contracted facilities to foster relationships. Serve as a role model for the healthcare team. Coordinate patient and family services and act as a patient advocate. Identify and mitigate safety hazards. Participate in community education and public relations. Ensure ethical clinical decision-making. Additional Details
