We are searching for a
Home Health Physical Therapist
that will provide evaluation and therapeutic services in the home. Care is directed at rehabilitation to maximize patient independence and adaptation in the patient's home. Schedule: Monday - Friday; 8:30am - 5:00pm
NO WEEKENDS REQUIRED!! Location: Sonoma County and occasional coverage to Marin County By The Bay Health follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include
(but are not limited to): Evaluates patient care needs on admission and on an on-going basis Plans and coordinates the treatment program based on the patient assessment and consultation with the physician Implements the treatment plan and makes revisions as required, under the physician’s plan of treatment Notifies physician and other appropriate agency personnel of changes in the patient's condition Documents patient assessment and on-going care concisely according to agency policy and standards of practice Education/Experience : Graduate of an accredited Physical Therapy Education program Doctor of Physical Therapy (DPT) degree One year's experience in a health care setting preferred Certificates, Licenses, Registrations: Current California license in Physical Therapy. Pay Range:
