Job Description The clinical pharmacist is responsible for providing pharmaceutical care services to clinic patients including medication therapy management, drug therapy consultation, chronic disease management, participation in transitions of care, and patient education and counseling. The clinical pharmacist will participate in clinic and pharmacy staff development, participate in patient care coordination meetings and be an active participant in clinical practice guideline review and development.
Responsibilities
Provide high-quality pharmaceutical care to ambulatory patients, including medication reconciliation, therapeutic selection, implementation, monitoring, modification, and dispensing with appropriate documentation and communication.
Provide comprehensive patient education and counseling on medication therapy and administration devices to promote safe and effective outcomes.
Collaborate with providers and other health care professionals to individualize drug therapies based on patient parameters and current literature, safety, efficacy, and cost.
Participate in medication therapy management, disease state monitoring, adherence monitoring, and drug therapy optimization.
Participate in interdisciplinary teams to develop and implement clinical plans of care, treatment protocols, and measures to reduce medication errors.
Provide care oversight for high-risk patients and assist with transitions of care, including medication reconciliation for hospital discharge when applicable.
Document drug information questions, drug utilization evaluations, and adverse drug reactions; participate in quality assurance indicators and continuous improvement activities.
Maintain clinical competency and provide teaching, supervision of trainees, and training of new pharmacists.
Participate in projects aligned with department and organizational goals and support Pharmacy and Therapeutics processes and formulary adherence.
Maintain knowledge of outpatient reimbursement models including MTM and pharmacist provider status; develop and present in-services to healthcare providers.
Qualifications
Graduate of an accredited College of Pharmacy; PharmD strongly preferred.
Licensed as a Registered Pharmacist in the State of Washington in good standing, or eligible for licensure.
Completion of an ASHP-accredited PGY1 residency required or equivalent practice experience; PGY2 residency preferred for specific practice areas (e.g., Oncology).
BLS for Healthcare Providers active certification, or required within 30 days of employment.
Minimum 2-3 years of clinical experience in a practice setting preferred.
Board certification or equivalent advanced practice specialty highly desirable; specialty-practice prerequisites may apply.
Professional and effective verbal and written communication skills; strong customer service orientation.
Ability to multitask, set priorities, work accurately and independently; capability to direct and supervise residents, technicians, interns, and students.
Maintain credentialing for reimbursement by insurance and governmental payers as applicable.
Ability to participate in professional organizations and meet current clinical responsibilities in the outpatient setting.
Unique Physical/Mental Demands, Environment And Working Conditions Requires routine drug education presentations, ability to adapt to changing priorities, work in an environment of interruptions, maintain up-to-date drug knowledge and guidelines, and precept students and residents. Competency in applicable collaborative drug therapy agreements is required.

Document and communicate medication therapy decisions, educate patients on medication use, and coordinate with providers to optimize therapy.
Engage in medication therapy management, chronic disease state monitoring, and disease/medication education to improve outcomes.
Collaborate on care plans, treatment protocols, and adherence initiatives; participate in quality improvement activities and medication-use evaluations.
Provide supervision and training for pharmacy residents, technicians, interns, and students; contribute to professional education and committee work.
