Location: Kent Campus Hospital Status: Full Time 80 Hours Shift: Days SALARY RANGE: 129,292.80 - 200,387.20 YEARLY General Summary

The Director of Care Management will lead and evolve care coordination, discharge planning, and patient throughput strategies across our health system. This role directs the daily operations of care management services, including case management, utilization review, social work, discharge planning, readmission reduction, and transitions of care including the longer-term optimization these functions. The Director will partner closely with clinical and operational leaders to design, implement, and evaluate data-informed strategies that improve patient flow, reduce unnecessary delays, improve health outcomes, and enhance the patient and provider experience. This role supports Bayhealth’s goals for high-quality, value-based care. Reporting to the Vice President / Chief Health Information Officer, this leader will also play a key role in aligning care management practices with system-wide priorities in quality, capacity management, regulatory compliance, and fiscal prudence —while maximizing the capabilities of the Epic EHR platform. Responsibilities

Lead / facilitates relationships with key external partners to manage and support care for patients across the care continuum. This includes payers, community resource groups, and government officials or workgroups. Develop and lead strategic initiatives to reduce length of stay, improve patient throughput, and enhance discharge efficiency across the system in both inpatient and observations settings. Leverage Epic tools to track discharge readiness, manage progression of care, and support decision-making with actionable data. Evaluate performance data and lead intervention to support desired performance. Partner with interdisciplinary teams—including nursing, physicians, and ancillary services—to improve coordination and communication during transitions of care. Facilitate system-wide collaboration to proactively address barriers to discharge and optimize bed capacity. Establish, monitor, and report on key performance indicators (KPIs), including avoidable days, LOS, readmissions, discharge disposition, and boarding metrics. Lead, create, execute, and evaluate evidence-based interventions to improve KPI performance. Ensure department practices are aligned with CMS, Joint Commission, and payer requirements for utilization review and discharge planning. Drive continuous process improvement and innovation in care coordination workflows. Support staff development, mentoring, and engagement to build a high-performing, patient-centered care management team. All other duties as assigned within the scope and range of job responsibilities. Required Education, Credential(s) and Experience

Education: Master Degree Nursing or Social Work, Healthcare Administration, or related field required. Credential(s): None Required Experience: Required: 7 years of progressive experience in care management in an acute care or comparable setting; 3 or more years in a related leadership role. Demonstrated success in implementing throughput improvement initiatives and care coordination strategies. Preferred: 5 years in a clinical managed care leadership position Preferred Education, Credential(s) and Experience
