In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) – Syracuse, NY
Base pay range: $95,738.00/yr - $206,206.00/yr Signify Health, part of CVS Health, seeks a Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations, engaging with people in their homes and helping bridge gaps in care. You will play a critical role in building trusted relationships to make people healthier by delivering flexible, comprehensive, and personalized health evaluations at customers\' front doors. Job highlights
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person\'s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You will use an iPad with an intuitive clinical workflow. You are not required to prescribe medication, order lab tests, or alter current treatment regimens. In This Role, You Will
