This is an on-site role with a physical working location of either: Anna Jacques Hospital in Newburyport, MA with the working hours of: 7:30am to 4:00pm OR Beverly Hospital in Beverly, MA with the working hours of: 8:30am - 5:00pm

Responsibilities

Under the direction of the Director of Health Information Management, handles various audit requests for the release of confidential patient medical information, coordinates the completeness of the documentation with the record requested, and performs a complete documentation review.

Coordinate logging and tracking of requests and the audit process from initiation through mailing, in collaboration with other hospital departments.

Organizes follow-up appeals documentation post-audit and coordinates the submission of the Physician Attestation Statements for Signature verification audits.

Assist requestors with access to confidential patient information in accordance with state, federal, and regulatory requirements governing privacy; assure confidentiality standards are met; provide accurate and timely medical records services; ensure customer/patient satisfaction for the Health Information Management Department.

Responds to requests for patient medical record information from various sources and verifies legitimacy of authorizations to meet regulatory requirements.

Coordinates with release of information services/agencies for photocopying & scanning to ensure accuracy and timeliness.

Apply HIPAA privacy rules and legal statutes; handle sensitive information per state and federal laws; direct issues to the HIM Director as needed.

Review audit requests for specificity and verify documentation; coordinate with physicians for completion when needed.

Work with Medical Staff, Physician Office Managers, Medical Staff Office, Patient Accounts, Case Management, etc. to obtain required documentation for audits.

Manage documentation reviews, completeness, and submission to Permedion and EHR for appeals; track for future reference.

Ensure all patient information is complete and signed prior to release; identify documents needing completion and coordinate with physician delinquency staff as needed.

Coordinate records needs for offsite storage, imaging, and other departments to comply with HIM policies.

Maintain confidentiality and a need-to-know basis; uphold privacy principles to prevent identity theft or unauthorized access.

Ensure secure access to department during business hours; maintain record tracking systems in EPIC and IatriTRAC for audits.

Adhere to HIM Department and hospital-wide quality assurance standards and promote patient privacy.

Follow hospital and department policies to contribute to safety, efficiency, and effectiveness of the HIM Department and hospitals.

Minimum Qualifications Education:

High School education or GED

Licensure, Certification & Registration:

N/A

Experience:

Minimum one (1) year acute care hospital Health Information Management experience preferred.
