Sheppard Pratt The Segue Residential Transition Program assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills. What to expect
You will coordinate assessments, community integration activities, planning, linkage, and advocacy. This position requires general and specific knowledge of crisis stabilization, mental health, community-based programming, and entitlement programs. Responsibilities
Supporting patients and addressing concerns as they arise, including resource coordination, appointments, transportation, skills training, entitlements, and money management. Maintaining accurate and complete documentation related to client needs and services provided. Implementing appropriate crisis stabilization techniques as necessary. Providing direct service for medication monitoring, including assessment, education, and assistance with filling pill boxes. Providing transportation to clients using personal or company-owned vehicles. Facilitating hospital visits to potential community placements and organizing activities to assess community readiness. A high school diploma or GED, bachelor’s degree preferred. Two years of experience. A valid driver’s license and safe driving record.
