About The Role Casa Esperanza seeks a dynamic, energetic individual passionate about delivering recovery and wellness-oriented community-based case management and care coordination for people living with addiction, mental health, and co-occurring disorders as part of our Casa Community Support Program. The Casa Community Support Program (CSP) addresses the health-related social needs of individuals living with substance use and behavioral health disorders, helping them thrive in the community. Peer Recovery Coaching provides support from peers with similar experiences to foster hope, explore recovery, and achieve life goals. It is a non-clinical, non-medical service; however, the Peer Recovery Coach collaborates with the Multi-Disciplinary Team to support individuals within our integrated care model, which includes primary care, mental health, and addiction treatment. The Peer Recovery Coach must be able to work independently to fulfill responsibilities and collaborate effectively with the care team. While the role differs from clinical staff, their contributions are equally valuable. This is primarily an in-person, community-based role that requires travel, providing transportation for patients, and in-office time. Essential Functions Deliver services in a linguistically appropriate and culturally competent manner that embraces diversity, including racial, ethnic, gender/gender identity, sex, sexual orientation, and disability (including intellectual disability). Develop recovery or wellness plans with clients that support their chosen recovery pathways, reflecting their strengths, needs, abilities, preferences, motivation, and readiness. Share plans with the care team as relevant. Provide emotional and social support to facilitate positive life changes and skill development, using evidence-based practices like motivational interviewing. Share personal recovery experiences and employ coaching and mentoring techniques to support client awareness of strengths and skills (recovery capital). Serve as a role model and mentor, demonstrating the possibilities of recovery openly and transparently. Support clients in making positive life changes, including connecting to treatment, developing recovery-supportive relationships, and utilizing community networks. Manage a caseload of approximately 22 clients, providing ongoing support and motivation. Assist with connecting clients to various services, including treatment, recovery support, employment, housing, and HIV services, within Casa and the community. Act as a recovery liaison, supporting clients in meetings with probation officers, social workers, etc. Advocate for clients within healthcare and social systems, teaching self-advocacy skills. Provide client orientation and support in achieving wellness plan goals. Coordinate with Reentry and Housing Case Managers as needed. Collaborate closely with the Multi-Disciplinary Team on care planning, assessments, and transitions. Provide temporary transportation assistance during client transitions. Deliver services in safe community settings, including homes, inpatient units, day programs, and support centers. Conduct outreach to reengage or maintain client engagement, including in-person and remote contacts over a 21-day period. Coordinate with Patient Access and Practice Management teams for service enrollment and reauthorization. Administrative & Quality Assurance Obtain client consent for wellness plans and share in Evolv, Casa’s electronic health record. Maintain accurate documentation of all client encounters and data in Evolv. Complete data collection and reporting per requirements. Attend supervision and relevant team meetings. Submit reports timely to payors, regulators, and Casa teams. Experience Required Bilingual Spanish/English required. High school diploma or GED required; exceptions possible. Certification as an Addiction Recovery Coach (CARC) or working towards it, through Massachusetts or other recognized certifying bodies. Knowledge of SUD and mental health treatment, recovery resources, supported employment, housing, and HIV services. Experience in outreach and engagement with vulnerable populations. Willing to share personal recovery story. Availability to work weekends and evenings as needed. Familiarity with social, health, and cultural issues affecting the Latine community. Ability to work independently and as part of a team. Understanding of harm reduction principles. Strong communication and interpersonal skills, crisis management skills. Good standing with relevant professional licensure, if applicable. Computer skills, including Microsoft Word. Schedule Monday to Friday, 8 am to 8 pm, with weekend availability as needed. 40-hour schedule, including 1-2 evening shifts.
