The SUD Counselor in a Substance Use Prevention Treatment (SUPT) Program is a role that combines clinical expertise, administrative proficiency, and a commitment to client-centered care. This role is integral in conducting ASAM assessments and managing daily billing in the Smart Care Electronic Health Record (EHR) platform. The ideal candidate will be a professional with a strong background in SUD treatment, proficient in ASAM criteria, and capable of navigating Smart Care EHR software. SUD Counselor reports to Program Manager. Responsibilities
Assessment: Conduct comprehensive ASAM assessments of the client\'s needs, strengths, and challenges, including mental health, substance use, and medical, social, and environmental factors. Create care plans focused on psychosocial rehabilitation and vocational goals. Planning: Use motivational interviewing and supportive counseling skills to develop personalized treatment plans that address clients\' recovery goals. Group Facilitation and Skills Training: Lead psychoeducational groups to teach recovery, relapse prevention, and independent living skills. Case management: Connect clients to appropriate resources and services, such as mental health treatment, substance abuse counseling, medical care, housing assistance, and vocational support. Monitoring and Support: Maintain regular contact with clients, offer support and crisis intervention as needed, and assist the primary therapist in increasing motivation to engage in treatment. Collaboration and Discharge Planning: Work with the treatment team to ensure coordinated care and keep referral partners informed of progress and discharge plans. Documentation: Maintain accurate and timely documentation of client progress and services provided in accordance with program and regulatory requirements and Drug Medi-Cal guidelines. Crisis Management: Respond effectively to crises and coordinate with emergency services when necessary. Other Duties As Assigned
Monitor cases by verifying clients\' attendance; observe and evaluate treatments and responses; advocate for needed services and entitlements; obtain additional resources; intervene in crises; provide personal support. Maintain clients\' records by reviewing case notes; logging events and progress. Communicate clients\' progress by conducting weekly interdisciplinary meetings and evaluations; disseminate results and obstacles to the therapeutic team and family; identify treatment influences. Improve staff competence by providing educational resources; balance work requirements with learning opportunities; evaluate the application of learning to changes in treatment results. License/Certification: CADC / SUDCC Certification or higher (required) Experience: 2+ years in Substance use/Behavioral Health (required) Current CPR/First Aid Certification (Required) Valid CA Driver's License Current Health Screening (Required) TB Clearance (Required) Strong interpersonal skills Excellent communication and listening skills Ability to remain calm in stressful situations Professional conduct always Ability to keep patient information confidential Company Description
