Job summary: Coordinates family and community activities at the center level to support the self-sufficiency, healthy living, and overall well-being of enrolled children and families. Provides resources and case management services and acts as a liaison between families, staff, the community, and other family-related services. Encourages family members to become advocates for their children. Responsibilities
Establishes and maintains an outreach and recruitment process, ensuring enrollment of eligible children for the assigned service area. Encourages and maintains parent participation in center-based programs and activities. Submits reports to the Family and Community engagement specialist as required. Maintains knowledge in all community programs, make referrals, and services to families. Assists families in utilizing community resources, including medical, dental, social, educational, and occupational. Maintains documentation of all families and children, including emergency information. Completes the PFCE Family Needs/Goals Assessment in the development of the Family Partnership Agreements with families. Recruits, supports, and documents all volunteer activities. Conducts Parent Meetings with families monthly. Conducts family home visits (both mandatory and as needed). Have knowledge of all Brazos Valley community action programs and resources to provide information, make referrals, and deliver integrated services. Monitors attendance of enrolled children, contact family when child has been absent for two consecutive days without any contact. Provide support to families of children with disabilities. Provide information on Special Education services as needed. Document and track follow up needs such as Family Partnership Goals, health, dental, nutrition, growth assessments, hearing, and vision concerns. Conduct a Family Interest Survey and submit it to the Nutrition Manager. Work with families to ensure child receives all required screens and exams within stated timeframe. Complete refusals (when or if needed) within an appropriate timeframe. Must be available for varied working hours to accommodate family schedules. Job responsibilities may vary depending on the program model, site needs, and locations. Qualifications
Must be at least 18 years of age. Minimum of a credential or certification in social work, human services, family services, counseling or a related field within eighteen months of hire. Preferred: Associate or Baccalaureate Degree in a related field and a minimum of one-year experience working with children and adults. Preferred: Experience with Head Start and/or preschool, Health and Human Services a plus. Successfully complete the Family Services Credential within one (1) year of employment. Must have a valid state license. Must be able to obtain CPR & First Aid certification within 30 days of employment. Position is contingent upon the successful completion of a federal background check. Physical Requirements
