May also oversee Prevocational and Respite Services. Ensure that individuals in the Day Habilitation Services receive appropriate day services. Participate in conducting activities for these individuals. Supervise staff within the program.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Supervise activities of Program Staff: carry out a broad range of supervisory leadership functions, including, but not limited to the following: managing and appraising staff performance; formal and informal training, orienting new employees, and other activities. Develop, implement, and schedule program activities and staff assignments: plan and implement a broad variety of activities related to habilitation goals and interest areas, including volunteer opportunities, community integration, self-advocacy, disability issues, and personal development. Participate with individuals and staff in activities as indicated. Provide ongoing guidance and direction to staff. Train staff in appropriate intervention techniques and DSP Competencies. Develop and implement staff action plans utilizing the HCBS waiver key of allowable services. Reviews the Regulatory Tracking Spreadsheets to ensure compliance and ensure that quality indicators are being met. May coordinate transportation services with appropriate personnel and/or provide backup for this function. Assigns drivers to various daily routes to and from the program. Coaches’ drivers regarding behavioral and medical support for participants while on the van runs. Ensures that AMAP staff are assigned to runs where a PRN may be required during transit. Arranges for substitute drivers when needed. Maintains Ready-To-Go binders in conjunction with nursing. In-service staff on the pre/post inspection sheet and the van bookbinder as required by site location. May serve as “Fire Marshall” for assigned sites, responsible for the coordination of quarterly drills and drills within 30 days of a new hire, evacuations, training, and completion of documentation as required by site location. Review fire evacuation and other emergency procedures with direct reports and discuss evacuation procedures for everyone. Assign staff to individual roles for evacuation and emergency procedures. Update fire evacuation plans when there are changes in staff or individuals receiving services. Formulate Staff Action Plans. Conduct individual assessments. Complete assessment summary, progress notes, data collection, etc. Chair team meetings; present Staff Action Plan progress and goals for the program. Review and certify staff action plans and monthly reviews (notes) completed by Habilitation Instructors and Specialists for quality indicators and billing requirements. Monitor and assign DSPs for daily data collection responsibilities and check daily the entries for compliance/ completeness. Generate and review billing reports for the program weekly and submit them to finance promptly. Responsible to complete or assign the daily attendance calls each morning for individuals who are absent and did not call in as per OPWDD guidelines. Serve as lead in the reporting of incidents. Follow up with Quality Improvement and the Program Director. Work with individual’s special needs during a variety of training activities designed to ensure individual safety, promote attainment of goals, and provide appropriate role models. Perform a variety of procedures as needed to fulfill each individual’s plan. Assist other staff members in taking individuals on and off vans, providing physical support, as needed. Serves as liaison with families, parents, and group homes. Ensure the program area is clean and safe. Ensure various supplies, equipment (medical/program supplies), and other materials are in good supply, are stored correctly, and are in good repair. May be required to work remotely and use technology or alternative means of instruction to provide remote services. May be required to prepare and implement lesson plans/activities and adjust instruction to accommodate the difference in the individual's needs via virtual sessions. Other duties may be assigned as needed.
Nonessential Elements
With Director, participate in meetings regarding program issues and/or projects of the agency. Perform a variety of clerical and administrative functions. Provide new individual orientation and conduct tours of the program. Perform ADL/personal care activities for individuals, as well as behavioral interventions, as needed. Provide transportation services by driving the agency van as needed. Additional responsibilities as directed
