Ennoble Care is hiring for a full-time Hospice Registered Nurse Case Manager for our hospice patients, who resides within or in close proximity to Passaic County, NJ. As a member of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing, the attending physician, and the interdisciplinary group to provide skilled nursing assessments, planning, and care to maximize comfort and health for patients and their loved ones/caregivers. Responsibilities include identifying patient and caregiver needs and delivering supportive care in accordance with the attending physician’s orders, plan of care, and agency policies and procedures. The functions of this position include the following: Shall be assigned patient caseload by the Patient Care Coordinator or designee Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process Initiate communication with attending physicians, Hospice staff, and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers Maintain regular communication with the attending physician concerning patient and caregiver needs Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support Provide accurate and timely documentation in electronic medical records Participate in and contribute appropriate information for Interdisciplinary Group meetings Coordinate physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate Meet regularly with the Hospice nursing staff to review issues from caseloads and share professional growth feedback Attend patient care conferences and facilitate discussion of caseload issues for staff discussion, consultation, and evaluation Supervise and document the NAC every two weeks or as assigned Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during on-call time or as requested Provide appropriate support at time of death and bereavement Participate in orientation and in-service training programs for professional staff Participate in agency and community programs to promote professional growth and understanding of hospice care Maintain familiarity with agency policies and state and federal hospice regulations Accept other assignments as appropriate Qualifications
Must be a graduate from an accredited School of Nursing Must be licensed in NJ as a Registered Nurse by the New Jersey State Board of Nursing in accordance with NJSA 45:11-23 et seq and NJAC 13:37 One year of RN experience; hospice experience preferred Excellent assessment and problem-solving skills Strong organizational and time-management skills; knowledge of case management processes Must be a licensed driver with an insured automobile in good working order Ability to communicate well, both verbally and in writing Fluent in English and Spanish Benefits
