short-term rehabilitation and transitional care , providing you with the opportunity to support residents during critical recovery periods. In this leadership role, you will be responsible for
ensuring compliance with resident care standards and DHS requirements

with in-house therapy and integrated care What You'll Do
Ensure compliance with
DHS regulations, care plans, and resident rights Oversee MDS processes, care planning, and required documentation Conduct daily rounds to monitor care delivery and quality standards Review nursing notes and ensure accurate, compliant charting Mentor, support, and evaluate RNs, LPNs, and CNAs Collaborate with residents, families, and interdisciplinary team members Support the DNS and leadership team during surveys and QA activities Qualifications:
RN degree from an accredited nursing program Current, unencumbered Oregon RN license At least
1 year of supervisory experience in a Skilled Nursing Facility Knowledge of SNF regulations, nursing practices, and EMR systems CPR/BLS certification Strong leadership, organization, and communication skills Must speak, read, and write English fluently Benefits:
