The clinical dietitian will provide evidence-based, patient-centered nutrition care through the application of the nutrition care process. Responsibilities include nutrition screening, assessment, diagnosis, intervention, care planning, monitoring, evaluation, discharge planning, and transitions of care. The dietitian will collaborate with the interdisciplinary team to ensure optimal patient outcomes and may participate in quality improvement projects, create educational presentations, and train dietetic interns. Core Competencies

Maintain thorough documentation of patients’ nutrition status in the electronic medical record using the ADIME format. Use critical thinking to determine patients’ clinical nutrition needs. Knowledge of culinary and menu operations to provide patient-centered care related to diet orders, allergies, sensitivities, and preferences. Be attentive and empathetic towards patients and their families to meet customer service needs. Be prepared to precept dietetic interns, utilizing ACEND’s training course. Position Responsibilities

Perform initial and follow-up nutrition assessments, provide education, and communicate with the care team across various units. Utilize validated tools and guidelines to assess for malnutrition, prioritize high-risk patients, and document findings accurately. Monitor patient weights, determine goals focused on patient-centered care, and serve as a mentor to staff. Conduct tray assessments, provide menu feedback, and ensure dietary accuracy using Computrition software. Assist with education, professional development, and adherence to practice standards and ethics. Participate in quality improvement projects and departmental policy development. Qualifications

Bachelor’s Degree in Nutrition (Master’s preferred for registrations after 2024). Registered Dietitian (RD) and Licensed Dietitian/Nutritionist (LDN) required. Experience in healthcare or LTC preferred. Computer proficiency and excellent communication skills required. Physical Requirements

Sitting, standing, walking, lifting, computer use, and exposure to cold vary in frequency as specified. Remote Type
