Clinical Pharmacist - Specialty Pharmacy (Ambulatory) position at LCMC Health. The role involves directing and initiating the development, implementation and maintenance of clinical pharmacy services, providing clinical consultation to the Medical Staff and healthcare teams, and ensuring high-quality patient care through effective collaboration and education. Responsibilities

Direct and initiate development, implementation and maintenance of clinical pharmacy services. Provide clinical consultation to the Medical Staff and other health care providers; suggest cost-effective therapeutic alternatives as needed. Communicate and document all activities/interventions (e.g., medication order changes/clarifications, pharmacokinetic dosing/monitoring, adverse drug reactions and errors). Foster teamwork and ensure delivery of high-quality patient care; perform staff duties as needed. Collaborate with health care providers to resolve concerns and questions; provide ongoing education and timely drug information to physicians, nursing, pharmacists, and other professionals; develop educational materials and competencies. Facilitate development and implementation of treatment guidelines, pathways, protocols, formulary changes, and drug information reports approved by committees. Participate in clinical committees; act as liaison to other practitioners; adhere to medical record documentation, JCAHO and CMS standards, and relevant policies. Support accreditation readiness activities and ensure compliance with ACHC and URAC Specialty Pharmacy standards. Lead and support prior authorizations and appeals; collaborate with providers, payers and patient access teams; stay current on payer policies and formulary restrictions. Ensure prescriptions align with 340B definitions and collaborate with 340B operations; document workflows impacting 340B compliance outcomes. Education/Experience Qualifications

Combination of appropriate education and work experience required. Hospital Pharmacy: Pharm.D. degree with 2 years of inpatient/hospital clinical experience or completion of Pharmacy Residency. Specialty Pharmacy: Pharm.D. degree with 2 years of experience in retail/specialty pharmacy and/or hospital/clinical capacity or completion of Pharmacy Residency. Licenses and Certifications

Currently licensed or registered to practice pharmacy in Louisiana. Board Certification in applicable specialty area preferred. Skills and Abilities

Adheres to LCMC Health rules and regulations; maintains standards per accreditation bodies (State Board of Pharmacy, Joint Commission, CMS, etc.). Maintains state license and continuing education requirements as appropriate. Proficient in computer systems to order within an Electronic Health Record; basic inventory management knowledge related to drug shortages. Ability to prioritize tasks, be self-motivated, and communicate effectively at all levels (written and verbal). Willingness to take on additional responsibilities as needed to support the department and other duties as assigned. Work Shift
