Clinicians visit patients at home, delivering continuum of care for individuals with chronic conditions and limited mobility. We are looking for an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in the Camden County, NJ region. You will apply medical techniques and principles under physician supervision to care for a diverse patient population with chronic diseases. What You’ll Do
Provide high-quality primary and palliative care to patients in and around Camden County, NJ Assess health through physical examinations, obtain and update medical histories, and develop care plans Communicate effectively with office staff and clinical operations across internal platforms Deliver patient-centered care to a defined panel of patients Attend quarterly medical staff meetings Develop impressions, diagnoses, and treatment plans Foster trust and consult with patients, families, and caregivers during visits Order diagnostic testing as medically necessary Prescribe medications, supplies, and medically appropriate interventions What You’ll Need
Board Certified with a valid NP or PA license in New Jersey Strong clinical skills to accurately assess, diagnose, and treat Positive demeanor and strong interpersonal skills Ability to work independently and solve problems confidently 1–2 years of primary care experience with geriatric patients (hospice experience preferred but not required) What You’ll Get
