Position Overview SCOPE OF ROLE:

The Wellness Coach will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral, and follow-up.

Minimum and Maximum Salary

Min:

USD $50,000.00/Yr.

Max:

USD $55,000.00/Yr.

Essential Duties & Responsibilities

Provide case management to assigned caseload

Develop and monitor individual service plans in conjunction with agency consumers

Assist agency consumers with problem solving and crisis intervention

Accompany agency consumers on medical, psychiatric, and public benefit appointments, as necessary

Act as advocate for consumer as appropriate; encourage and support self-advocacy

Identify problems in daily living skills; provide hands-on assistance and/or referrals

Support Facilities Management staff in maintaining apartment standards as outlined by S:US, OASAS, and other funders

Provide supportive counseling to agency consumers as needed

Conduct regular home and hospital visits as necessary

Maintain strong working relationships with therapeutic support services

Document all services in compliance with agency standards

Assist with moves, including lifting, packing, and cleaning

Participate in community activities and develop off-site recreation, socialization, educational, and skill-building activities

Liaise with community-based services and groups

Participate in supervision, team meetings, case conferences, and trainings

Adhere to on-call system for crisis intervention and emergencies

Perform other duties as assigned

Qualifications Required Education and Experience

Bachelor’s degree in Social Work or related field with two years’ experience in case management or working with the target population

Experience with mental illness, substance use, homelessness, or related human services

Experience in group work, tenant organization, referral, or outreach preferred

Experience or training in peer counseling or advocacy preferred

Proficiency in computer programs including email, Microsoft Word, Excel, PowerPoint, and Windows OS

Preferred Skills

Bilingual in English/Spanish preferred
