Overview

Physician - Hospice position with Housecall Providers, a Portland area nonprofit focused on delivering home-based medical care. The hospice physician provides professional, comprehensive medical care and symptom palliation, primarily in home or facility settings, and may advise and direct the interdisciplinary team. This is a part-time (0.5 FTE) hybrid role with specific weekday scheduling and limited evening/on-call coverage. Responsibilities

Certify patient eligibility for hospice services and recertification for continuation of Medicare Hospice Benefit. Collaborate with the interdisciplinary team and the patient’s attending physician to create, implement, and revise care plans aligned with patient goals and needs. Attend IDG/IDT meetings as medical resource, guiding the medical services team to ensure quality standards of care. Provide consultation, support, and education to the hospice IDG, attending physicians, and colleagues on medical eligibility and symptom management; ensure 24/7 medical services availability directly or through arrangements. Document care in the patient’s clinical record, including progression of end-stage disease and opioid needs to meet CMS/DEA requirements. Prescribe and manage medications for symptom control; initiate or adjust medical regimes for new patients; review medications for necessity and cost-effectiveness. Provide compassionate anticipatory guidance and high-quality palliative care focused on patient and family goals; participate in family meetings as needed. Assess hospice care needs, determine referrals for specialty or rehabilitative services. Manage complex cases with independent judgment and ensure delivery of care while meeting productivity metrics. Provide oversight of care delivered by Nurse Practitioners as directed by Medical Director; cover for clinical colleagues as needed. Participate in 24/7 consultation and care support, including overnight and weekend coverage (primarily telephonic). Administration

Collaborate with medical and mental health professionals and technical staff; participate in conferences and staff development activities. Attend required staff meetings and ad hoc committees; participate in quality improvement activities. Complete and maintain medical records per agency protocols and support quality metrics; may train/assess other providers as assigned by the Medical Director. Help ensure continuity of hospice medical services and contribute to updating patient care policies and emergency procedures. Address interpersonal conflicts and clinical/ethical concerns with professionalism; stay current with hospice care research and pain/symptom management trends. Organizational Responsibilities

Required

Licensed physician (MD or DO) in the State of Oregon Board certification or eligibility in hospice and palliative care by the American Board of Medical Specialties or equivalent DEA license Valid driver’s license or access to insured vehicle CPR certification at hire or within 6 months Preferred

Experience in Internal Medicine or Family Medicine Experience in geriatrics and providing primary care for hospice patients Experience with home visits and working with safety-net populations on mental health, substance abuse, and behavioral health issues Experience with medical ethics and quality improvement methodology Knowledge, Skills And Abilities

Knowledge

Ethical and medico-legal aspects of medical practice; public health principles; understanding of hospice regulations and HIPAA Knowledge of the population served in adult health, including cultural and demographic considerations; familiarity with palliative interventions and medications Skills And Abilities

Current clinical and regulatory knowledge for hospice eligibility; ability to work with dying patients in hospice; strong communication and conflict resolution skills Leadership capacites to organize and motivate teams; effective communication with diverse staff and families; problem-solving and EMR proficiency Ability to work independently and collaboratively; manage multiple priorities; adapt to flexible scheduling including evenings and weekends Physical ability to perform job duties (e.g., mobility, lifting, driving) per job demands Working Conditions

Estimated Hiring Range: $222,750.00 - $272,250.00.
