Overview
Geriatric Medicine Nurse Practitioner opportunity with Optum in North Carolina. The role focuses on providing home-based wellness visits for older adults, bridging gaps in care, and delivering wellness-based care plans.
This role offers complete autonomy, and you will work on a team of Nurse Practitioners that is managed by Nurse Practitioners. The platform delivers wellness-based care plans in the patients' homes. Many patients do not see their PCPs, and you are the provider who bridges that gap.
You can create your own schedule between 8:00 a.m. and 8:00 p.m., Monday through Sunday, and you may work an eight- or ten-hour day. Optum will never require nights, weekends, holidays, or on-call shifts. You will have up to one hour with each patient. Charting is completed during visits on a tablet provided by Optum.
Responsibilities
See 5-6 patients per eight-hour day or 7-8 per ten-hour day conducting Annual Medicare Wellness Exams.
Complete a physical examination, head-to-toe assessment, medication reconciliation, and risk assessment.
Identify gaps in care, educate patients on nutrition, and document any findings or testing recommendations.
Chart during visits on a tablet provided by Optum.
Work as part of a team of Nurse Practitioners; contribute to wellness-based care plans delivered in patients' homes.
Compensation and Benefits
Base salary with quarterly bonus payable monthly, with a 20% cap.
Sign-on bonus.

CME stipend of $2,500.
