Overview The Clinical Pharmacy Manager – Ambulatory Cardiology provides leadership and strategic oversight for cardiology-focused pharmacy services across ambulatory care settings at Montefiore Medical Center. With a strong emphasis on Collaborative Drug Therapy Management (CDTM), this role drives the development, implementation, and evaluation of pharmacist-managed cardiology clinics and telehealth services, ensuring the delivery of safe, effective, and patient-centered cardiovascular care.

Responsibilities

Reporting to the Director of Clinical and Educational Pharmacy Services and supervising two ambulatory clinical pharmacists.

Accountable for improving pharmacotherapy outcomes through direct patient care, interdisciplinary collaboration, provider education, and participation in outcomes research and cardiology-focused performance improvement initiatives.

Provide leadership and strategic oversight for cardiology-focused pharmacy services across ambulatory care settings.

Drive the development, implementation, and evaluation of pharmacist-managed cardiology clinics and telehealth services.

Qualifications

Education & Licensure Requirements:

Doctor of Pharmacy (PharmD) degree required

Active license to practice pharmacy in the State of New York

Board Certification in Pharmacotherapy or equivalent certification preferred

Eligible for credentialing as an Allied Health Professional

Experience:

Completion of a PGY2 Residency in Cardiology is strongly preferred. In lieu of a PGY2, completion of a PGY1 Pharmacy Residency and a minimum of 3 years of direct clinical experience in cardiology or ambulatory pharmacy practice is required.

Demonstrated expertise in disease management (e.g., heart failure) and preventive cardiovascular care in adult populations.

Proficiency in geriatric pharmacotherapy principles, including deprescribing, fall risk mitigation, renal dosing, and polypharmacy management.

Experience in clinical research design and management.

Proven ability to balance patient care, education, and departmental initiatives in a dynamic environment.

Strong interpersonal, leadership, and team collaboration skills.

Working knowledge of data analytics tools (e.g., Web Intelligence, Clinical Looking Glass) and electronic health record systems (e.g. Epic).

Occasional travel to other Montefiore Medical Center sites may be required.
