Overview
Lowell General Hospital is currently seeking an outpatient Cardiology Nurse Practitioner to join the Heart Failure Program team within the ConnectCare Hub in Lowell, MA. This part-time position will be 24 hours per week with 8-hour shifts. This role will provide advanced clinical care to patients with heart failure in an outpatient setting, collaborating with cardiologists, primary care physicians, nurses, and other healthcare professionals. The role will manage complex heart failure cases, optimize therapy, reduce hospital readmissions, and improve patient outcomes and quality of life.

The Heart Failure Clinic provides comprehensive and individualized disease management, education, support, and follow-up to patients with advanced heart failure, with the aim to manage symptoms early and prevent avoidable hospitalization. Cardiac nurse practitioners work to optimize and transition the patients’ care by intensely managing medications, coordinating care with home agencies, and using telehealth monitoring at home to detect early signs of decompensation. The team provides immediate responses to acute concerns with telephonic medication management, clinic urgent visits (provider visit and on-site IV Lasix), and utilization of Mobile Integrated Health with paramedic visits and in-home IV Lasix as needed.

Who you are
Graduate of an accredited and/or approved Physician Assistant or Nurse Practitioner program
Current license as a Physician Assistant or Nurse Practitioner in the Commonwealth of Massachusetts
Prior advanced heart failure or acute care experience preferred
Current certification in CPR required
A team player who is excited to work cooperatively with a wide range of stakeholders and disciplines to champion world-class patient satisfaction and support
