The Integrated Emotional Wellness Clinician is accountable to the Lead Integrated Emotional Wellness Clinician. This full-time role supervises Licensed Graduates and Master level interns and is based at the Conway Health and Resource Center in SW DC. Salary: $75,000 - $85,000. The offer amount is determined by education, qualifications, and experience; salary estimates from external calculators are not affiliated with COH. Responsibilities
Support and coordinate mental health care with the patient’s primary care provider and other treating mental health providers as appropriate. Screen and assess patients for mental health and substance use disorders; consult with supervisor on difficult cases. Identify and address related health determinants (housing, vocational rehabilitation, social integration) and coordinate services with center staff and community providers. Provide crisis intervention per licensing and COH guidelines, including addressing suicidal/homicidal ideation. Educate patients about mental health and substance use issues and available treatment options. Monitor patients for changes in clinical symptoms and treatment side effects or complications (in person or virtually). Provide brief, evidence-based interventions such as Behavioral Activation, Motivational Interviewing, or other appropriate primary care modalities. Deliver short-term, problem-focused therapy for up to 5 patients (up to 8 sessions per patient with supervisor approval) focusing on goal setting and brief interventions. Complete treatment plans by the second session and monitor progress, reevaluating and updating plans monthly. Collaborate with PCPs and psychiatric providers to support patients not improving as expected, including treatment plan changes or referrals as needed. Maintain and document all patient encounters in the medical record in a timely manner. Perform other duties as assigned by supervisor. Requirements
Graduate-level license or higher in social work, counseling, or related field. LGSW/LICSW, LGPC/LPC, or comparable license required; Maryland-licensed providers may be eligible for MD licensure reimbursement with required documentation. Minimum of two years of clinical experience preferred. Strong organizational and collaborative skills. ExperienceScreening for mental health symptoms required; screening for substance use disorders preferred. Experience with assessment and treatment planning for mental health and/or substance use disorders required. Familiarity with brief, structured intervention techniques required. Experience working with underserved populations preferred. Flexible hours including evenings and weekends; ability to travel between sites. Proof of vaccinations required; reasonable accommodations may be requested per law. On-site work designation with potential changes; remote work may be limited and is subject to change.
