Monday-Friday 8:00am to 5:00pm, occasional weekend events As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. The bereavement counselor will work under the direction of the Executive Director and/or the Regional Counseling Supervisor. Responsibilities Responsibilities include providing counseling and supportive services to meet the emotional and social needs of patients, families, and communities affected by terminal illness and loss; working under supervision to coordinate care and support in alignment with organizational guidelines. Qualifications Master's or Bachelor's degree in counseling, psychology, nursing, social work, family studies or other mental health discipline, certified/licensed per state law. Minimum of three years supervised experience in health or related field. Clinical experience in related field. Ability to exercise judgment and discretion in confidential matters. Knowledge and commitment to Bluegrass Care Navigators’ philosophy. Understanding of grief response and experience with counseling individuals and/or within groups regarding loss. Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies. Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
