The purpose of this position is to provide the services of a licensed pharmacist, which includes the responsibility for the safe and proper distribution and use of medications in inpatients and outpatients who range in age from neonates to the elderly. The pharmacist performs at a high level of clinical decision-making, proactively influencing drug therapy by making recommendations at the point of prescribing or, in some cases, making independent therapy decisions in collaboration with other healthcare team members. Additional services include medication reconciliation, discharge counseling, and coordination of care from admission to discharge. The pharmacist must possess strong communication skills for patient education and work collaboratively with inpatient and outpatient interdisciplinary teams, including providers, nurses, case workers, pharmacists, and technicians, as well as contributing to the teaching and precepting of students, interns, and/or residents. This position reports to Inpatient Central Operations leadership and supports both centralized operational functions and decentralized clinical services, with opportunities to cross-train and provide coverage across other inpatient service lines based on operational needs. The model will continue to evolve as part of an integrated decentralized patient care practice. Responsibilities
Identifies, resolves, and documents complex therapeutic problems, applying advanced knowledge to a specific patient population. Responds to requests for drug information from physicians, nurses, patients, and families; independently reviews and interprets literature to apply to patient care situations. Monitors therapeutic response and adverse effects of medications; consults with providers as needed and documents activities in the patient medical record. Reports adverse drug reactions, drug defects, and medication errors. Supervises activities of technicians, students, interns, and residents; monitors and evaluates their performance. Influences standards of therapy by building peer-level relationships with attending medical staff; is recognized as a drug authority within practice areas. Assists in developing treatment guidelines and documents activities in patient records, pharmacy monitoring profiles, and related data collection or reporting programs. Interprets and evaluates physician orders for accuracy and appropriateness; documents clinical decisions or distribution issues requiring follow-up or continued monitoring. Verifies accuracy of medications prepared and dispensed by the central pharmacy. Participates in intra- and interdepartmental education programs and contributes to peer development. Assists in reconciliation of medications, patient education on home regimens, and coordination of discharge medication plans with the interdisciplinary team. Documents activities in the patient medical record. Required Qualifications
A current Pharmacist’s license issued by the Oregon State Board of Pharmacy. PGY1 Practice Residency or equivalent experience (defined as 3 years of direct patient care with at least 50% time in pharmacotherapy activities as defined by the Board of Pharmacy Specialties). BS in Pharmacy or Pharm.D. Job Related Knowledge, Skills And Abilities (Competencies)

The pharmacist will experience competing demands from nurses, physicians, pharmacy staff, and other healthcare professionals. The role requires flexibility to support both centralized operations and decentralized clinical services, including schedule coverage from Monday to Sunday and overnight shifts. Staffing assignments may vary based on operational needs. Benefits
