The Senior Pharmacy Clinical Specialist is responsible for performing expert clinical pharmacist duties to enable optimal pharmacotherapy, promoting safe, efficacious, and pharmacoeconomically rational use of medications, and conducting daily medication therapy regimen reviews within designated areas or populations. The role includes education for pharmacists, nursing and medical staff, CQI evaluations, and basic research. It also includes precepting pharmacy students and residents on rotations and contributing to the organization’s mission of innovative, personalized, cost-effective pharmaceutical care in a quality- and safety-focused culture. Responsibilities
Promote a positive, team-focused work environment. Collaborate with other health care professionals for problem resolution and patient-specific goals. Mentor Clinical Specialists and pharmacy residents on research projects. Precept ASHP-accredited residency program residents and students from affiliated colleges of pharmacy. Develop educational materials and provide educational programs for pharmacy staff and other hospital personnel. Service Essential Functions
Process orders/requests accurately and efficiently, verify drug appropriateness based on diagnosis, allergies, weight, and other factors. Document patient care activities and outcomes related to pharmaceutical care and clinical interventions. Provide patient-specific and non-patient-specific drug information support regularly. Lead policy and procedure reviews and respond to audit reports within areas of responsibility. Serve as a member of Medical Staff committees and contribute to committee goals. Quality/Safety Essential Functions
Develop medication therapy/disease state management protocols and dosing guidelines within the specialty area. Provide pharmaceutical care at an advanced to expert level, including proactive drug therapy reviews, patient counseling, transitions of care, and management of pharmacist-based dosing protocols. Lead internal quality management and research programs; develop, implement, evaluate, and report QA outcomes; conduct pharmacy research aligned with departmental goals. Serve as an on-call resource in assigned roles as determined by manager (e.g., ID Stewardship, Investigational drugs, or general clinical). Finance Essential Functions
Contribute to evaluating medications for the formulary and provide pharmacoeconomically sound medication decisions; participate in committees and initiatives to optimize pharmacoeconomic benefits. Independently manage time and prioritize tasks for self and the specialty area. Growth/Innovation Essential Functions
Contribute to intra- and interdepartmental publications; support ACPE delivery through programs and resident preparation; participate in research projects to advance department goals; submit work for publication and presentations. Provide presentations at local, state, and national meetings and in-services to pharmacy, medical, nursing, and nutrition staff; seek opportunities for continual development as a Senior Pharmacy Clinical Specialist. Graduate of an education program approved by the credentialing body for required credentials in the Certifications, Licenses and Registrations section. Work Experience
Must have one of the following: ASHP-accredited PGY-1 and PGY-2 residency programs or equivalent experience (three additional years of pharmacy experience per residency year); or completion of a pharmacy fellowship with applicable focus may be considered equivalent to a PGY-2 residency. Six years of clinical practice experience. Three years of experience as a HM Clinical Specialist. Meets HM standards of an ASHP-qualified residency preceptor. Licenses and Certifications - Required
Texas Pharmacist licensure or eligible within 90 days. Pharmacist Preceptor Certification within 90 days. Basic Life Support (BLS) or Instructor within 90 days. Advanced Cardiac Life Support (ACLS) or Instructor. Licenses and Certifications - Preferred
Pediatric Advanced Life Support (PALS) or Instructor; IV Certification; Board of Pharmacy Specialty certification. Knowledge, Skills, and Abilities
Demonstrates skills and competencies to safely perform the assigned job with ongoing evaluations. Proficient English communication for patient safety and operations. Strong interpersonal, teamwork, and leadership abilities; coordinates activities of a major pharmacy service area. Professional and technical competence in drug selection, evaluation, utilization, and information for all patient groups (neonate to geriatric). Proficiency in rational drug therapy and evidence-based guidelines; experience with direct patient care and collaboration with nursing/medical staff. Knowledge of operational pharmacy practice and research experience related to pharmacy practice; maintains credentialing with HM Academic Institute as applicable. Research competence in pharmacy practice. Supplemental Requirements
