The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions. Hours of operation : 8:00am-7:00pm; Monday - Saturday Schedules Available : Monday- Friday, or work Saturday with the option to take one day off during the week. Shifts Available : Full Time; 10:30am - 7:00pm Expected starting wage : $26.46 - $32.69 per hour. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. What You Bring to the Table (Must Have) High School Diploma or G.E.D and a relevant certification such as CADC, CATC, SUDCC (I-IV), or CDP (in WA). In AZ, must be certified as one of the required state credentials (LISAC, LASAC, LSAT). Number of Units : 75 Who We Serve : Adults experiencing chronic or literal homelessness with a disabling condition substantially limiting one or more major life activities (HUD Category 1). Hours of Operation : Monday - Friday, 8:00 a.m. – 7:00 p.m. About the Program : In partnership with San Diego Behavioral Health Services and San Diego Housing Commission, Telecare offers onsite supportive and case management services for residents at The Shores and Pacific Village. Services focus on housing stability and retention and are provided based on preference and need, including on-site, in units, or in the community. Service activities include: outreach and engagement for housing stabilization and case management; behavioral health assessment, screening, care coordination, and referrals for residents at risk; ongoing behavioral health care coordination; and site-wide community integration opportunities.
